COVID-19 CONSENT AND WAIVER

I,, acknowledge that Shahrzad Tayebi (Lactation consultant, Craniosacral therapist), in returning to work in this unprecedent condition of COVID-19, has confirmed to me that she has adhered to all health standards and guidelines set by the health authorities, she has not travelled out of Canada in the last 14 days and has not been sick or knowingly in contact with a person with confirmed COVID-19.
I confirm that I am not currently showing any symptoms of COVID-19 (fever, coughing, shortness of breath, difficulty breathing, flu like symptoms, etc), and I have not, to my knowledge, contracted COVID-19.
I confirm that I have not travelled out of Canada in the last 14 days, nor have I been in close contact with someone travelling from out of Canada in the last 14 days.
I confirm that I have not had close contact with a person with confirmed COVID-19 (Note: This means you would have been contacted by your health authority’s public health team.)
These terms also are for anyone in my household.
I understand that the consultation/treatment will be cancelled immediately if the client does not meet all the pre-screening criteria.
I understand that while Shahrzad Tayebi is following all of the health and safety guidelines outlined by the Health authorities and they are taking all reasonable precautions to mitigate risk, there are no guarantees that I may not come into contact with COVID-19.

By accepting this form, I acknowledge that I am aware of the risks involved and give consent to receive consultation/treatment.
